Partners Health Plan (PHP), a not-for-profit managed care organization, is dedicated solely to providing supports and services for persons with intellectual and other developmental disabilities (IDD). It was founded by AHRC Nassau and is now governed by all five Downstate AHRCs/ARCs. PHP was developed by the same providers who have effectively served individuals with IDD in the community for decades and we are committed to employing a person-centered approach to care planning and service delivery that addresses the needs of the whole person. Planning and developing this model has taken several years, and PHP is the culmination of a rich and robust collaboration among a diverse group of providers and other stakeholders dedicated to serving this population.

Partners Health Plan (PHP) is a not-for-profit managed care organization serving individuals with intellectual and other developmental disabilities (IDD) and those who care for them. With more than six decades of experience serving persons with IDD, PHP has the expertise to develop and coordinate a personalized Life Plan that ensures participants receive the essential medical, behavioral, dental, habilitation, and other services and supports needed to support each participant’s health, safety, personal preferences, and valued outcomes. We understand the needs of our participants and we work closely with local providers and community groups to meet those needs.
We also know that health care can be confusing. That’s why a highly qualified care management and coordination team will be assigned to help you and your family or caregiver.
